Change of Name Appointment Booking

Personal information

Hertfordshire County Council will use the data you provide for the sole purpose of making your appointment with the Registration Service. Your personal details will be retained for a maximum of 2 years and will not be passed to any other parties or used for any other purpose.

Cancellation policy

Any cancellation made on the day of the change of name appointment may result in no refund being given.

Applicants between 16 & 17 years of age

If you are subject to any of the court orders listed  below, you will  need consent to change your name from everyone with parental responsibility for you as long as the court order remains in force.

  • Child Arrangements Order in England & Wales which includes arrangements relating to who the child is to live with, or when the child is to live with any person
  • Residence Order in England, Wales, or Northern Ireland
  • Special Guardianship Order in England & Wales
  • Care Order in England, Wales, or Northern Ireland
  • Interim Care Order in England, Wales, or Northern Ireland

If you are subject to one of the court orders listed above (which is in force at the time you apply for the name change) and someone with parental responsibility doesn’t give their consent, you would need to apply for a court order allowing the change.

If any other court order forbids your name from being changed, regardless of parental responsibility consenting to your decision to change your name and for as long as the court order remains in force, then you’ll also need to apply for a court order (a Specific Issue Order) allowing you to change your name.

Foreign Nationals and British Nationals living overseas

British Nationals who live in other countries should first check with the relevant authorities in their country of residence that the deed will be accepted. Some authorities may require the document to be counter signed by a solicitor.

Hertfordshire County Council cannot accept responsibility if your country of nationality declines the use of the change of name deed you have been issued by us.  It is therefore advisable for foreign nationals, residing in the UK, to first check that their country of nationality will accept our change of name deed.
